Program Overview
The FAMILY program expands the children, youth, and adolescent crisis continuum by delivering rapid-response, community-based crisis intervention services for youth and families during critical transitions in out-of-home placements.
Purpose
- Support youth and caregivers during foster care or kinship placement
- Stabilize crises quickly during placement transitions
- Promote long-term emotional well-being and placement stability
Who Is Served?
- Youth entering foster or kinship care
- Youth and caregivers at any stage of placement
- Families experiencing placement disruptions or transitions
Key Services
- Rapid Crisis Response: Immediate support during high-stress transitions
- Crisis Stabilization: Community-based intervention to assist in acclimating to transitions
- Safety & Well-Being Assessment: Evaluation of emotional and physical needs
- Health Literacy Support: Guidance for caregivers and youth
- Short-Term Therapeutic Support: Up to 45 days of services during adjustment

Trained professionals work closely with youth and caregivers using a crisis intervention model to:
- De-escalate immediate concerns
- Strengthen caregiver capacity
- Support healthy adjustment to new living environments
- Deliver family-defined, individualized care
