The ACCESS Unit is committed to providing professional, affordable, and timely emergency, assessment, clinical mental health and substance abuse treatment services to individuals and families in our community.

Our Goals:
To provide consumers, clinicians and referral sources with easy access to services;

To quickly and effectively determine client needs;

To connect individuals with the appropriate level of service;

To ensure all clients receive appropriate, quality care based on “best practice” behavioral health care

Emergency Services: Staff provides 24-hours-a-day, 7 days a week availability for phone and face-to-face intervention which includes: crisis intervention; pre-screening for hospitalization; working with the legal, judicial, healthcare and other referral sources; and securing inpatient beds or alternative services.

Assessment Services: Trained clinical staff provides immediate response by phone (during specific hours).  The screening includes: establishing the nature of the request for services; answering questions or providing information about services and other resources; determining whether an emergency exists; collecting financial data; scheduling clinical intakes; and, obtaining initial authorization from insurance providers, if necessary.

Clinical Intake Services: During the intake process, staff conducts a thorough clinical assessment to determine the client’s service needs and/or appropriate level of services. A financial assessment is completed to determine payment responsibilities. An initial appointment is scheduled with the appropriate service provider and a follow-up is made to ensure the appointment was kept, the linkage was appropriate, and the client is satisfied.

Outpatient Services & Short-Term Counseling Services: Staff provides psychotherapy, education, crisis intervention and case management to clients with a wide variety of mental health and substance abuse problems.  Most of these services are provided in the community-based clinics, though some are provided at other locations. Services are individualized for each client/consumer and may be provided using individual, family or group treatment models.  Short-term counseling is available for clients who need immediate, on-going counseling beyond the initial intervention and prior to being able to set up entry into other services.

Stepping Stones - Intensive Outpatient Services: This program is designed to provide what an individual would get in terms of education and therapy from a traditional residential substance abuse program while allowing the person to stay at home and continue with his or her regular job.  The group meets three nights each week for ten weeks.  Each night’s session is for 3 hours.  The maximum number for each group is 12 people.  The group is open-ended so that people can join during any part of the ten-week cycle and gain full benefit.  Individual counseling is a concurrent requirement.

Relapse Prevention: This service provides education and treatment to individuals who have a history of relapsing into chemical use. Most of the service is provided in a group format but individual sessions are included.  Screening is done by Relapse Prevention Group Leader.

Residential Substance Abuse Services: The residential rehabilitation services at New Life Recovery Center provide treatment for chemically addicted individuals in a residential environment so they can be removed from the situations in which they are most likely to relapse, at a time when they are the most vulnerable. The program provides individual and group therapy for the clients, as well as education about addiction and case management. This includes planning to get them back into an environment that will be more supportive of their decision to stop using drugs. There are 12 beds for Intermediate Rehab services at this facility, which is located in Fairlawn.

FLASH: Families Learning And Supplying Help is a group provided for families of individuals with substance abuse problems.  The group meets once a week for 2 hours.

New Horizons Crisis Stabilization: This program targets adults with a primary diagnosis of mental illness (or those with a co-occurring diagnosis of mental illness and substance abuse) who are experiencing an acute psychiatric crisis. Unlike many situations where a client is hospitalized, persons who enter the New Horizons program must be competent and willing to accept admission. Services at New Horizons include psychiatric and diagnostic assessments, as well as medication evaluation/administration, individual crisis counseling, group counseling, peer support, case management, transportation during treatment and supervision, and safety monitoring.

Services for Adults on Probation and Parole: These services are offered through two situations. Clients referred by Federal Probation and Parole are assessed by staff who then provide services or refer to other agencies for help. NRVCS also has a contract with Probation and Parole for an early intervention program (EIP).

Testing Services: Under contracts and by referral, staff provides specific testing or assessment batteries including: forensic evaluations, disability testing and special education school testing.

Raft Crisis Hotline: Volunteers provide phone empathy, information and referral, reassurance and crisis work, for multiple types of problems with all ages and populations. The volunteers provide phone back up to ACCESS.  The hours vary depending on staff coverage. Find out more about Raft

Valley Employee Assistance Program: Program staff develops and markets contracts with business, industry and other agencies. Services include: assessment, crisis intervention, brief treatment and referral, if necessary, for contract employees and their family members.  Training for supervisory staff and orientation for employees are also included in the contracts.

ADULT & FAMILY SERVICES

The Adult & Family Services Unit focuses on providing therapeutic treatment, case management, support services, employment services, housing assistance, social and recreational skills development, outreach and referral to adult clients and their identified family support system within the community or our facilities. Staff responds to the individual’s needs through a multi-disciplinary approach and with a wide diversity of expertise. Families are assisted with education and other services to help them understand and support the goals of their family members.

Medical and Psychiatric Services: Nursing services provide the overall implementation of medical services. Nurses are the primary liaison with doctors, pharmacies and the labs. Nurses provide medication education to consumers, give injections, distribute state pharmacy medications, and provide medical information and training for other staff. Individuals must be a current client in the agency and linked to nursing services by the case manager or outpatient clinician in charge of the case.

Community Support Services
PACT (Program for Assertive Community Treatment) provides services to clients who, because of their mental illness, are either unable or unwilling to participate in services typically provided in an office setting. Instead, services are provided in the client's home by a team of staff clinicians, case managers, a psychiatrist and peer support technicians. The team monitors medications, helps with daily living skills, provides counseling and helps the client remain in the community, with the primary purpose of keeping them out of the hospital.

Case Management Services are provided for individuals with serious mental illness and/or developmental disabilities. Services include: discharge planning; residential supports; referral/accessing/and monitoring all facets of life; crisis intervention; family support and education; community networking and education.

Intensive Case Management Services are focused on adults with developmental disabilities and/or serious mental illnesses who also need help obtaining and/or maintaining residential and/or Medicaid waiver services.  Staff providing this service use the same interventions as case managers but caseloads are smaller, allowing more intensive work.

Community Day Support Services (Site and Community Based) provides psychosocial rehabilitation and day support services primarily to adults with serious mental illnesses and/or mental retardation. The focus is teaching and maintaining activities to help consumers lead productive lives. These services include Against All Odds Clubhouse, Extended Clubhouse Operations and Day Support Services. Individuals must first be active with case management or supported living services to enter one of these programs.

GMB is a mini-business operated by Clubhouse staff. Consumers work for competitive wages. GMB provides lawn care and cleaning services, both residential and commercial. Individuals must be screened by Clubhouse staff to enter this program.

Community Living Services
Group Homes (Fairlawn House and Elmtree) provide 24-hour congregate, residential care to people with developmental disabilities. Services include: locating housing; daily living skills; developing social skills; behavioral supports; transportation; day support; money management; leisure and recreation opportunities; community and social integration; medical and psychiatric support; respite; and, crisis intervention.

Supported Living Services provide intensive, individualized residential support services to people with developmental disabilities and/or serious mental illnesses. Services include: locating housing; daily living skills; developing social skills; behavioral supports; transportation; day support; money management; leisure and recreation opportunities; community and social integration; medical and psychiatric support; and crisis intervention. Supports vary from a few hours a day to 24 hours a day.

CHILD & FAMILY SERVICES

The mission of Child and Family Services is to support, strengthen, and promote healthy growth and development in children, youth, families and communities throughout the New River Valley.

Early Intervention Services: Staff provides service coordination for children 0-2 with developmental delays and their families. This includes: parent partnerships; assessment planning; IFSP (Individual & Family Service Plan) development; linkage and referral; transition planning; and, advocacy. Services are primarily provided in the home setting.  Case management is provided for children 0-6.

Prevention and Education Services: The major areas of programming for these staff include:

Peer Helper Program and Conflict Mediation - training of trainers on topics related to mental health, mental retardation and substance abuse;

Community Partnerships - A youth and adult partnership planning model aimed at reducing substance abuse issues in the community.

Clinical Services - Outpatient Services:
Individual Counseling - Staff provide individual therapy to those at risk of or diagnosed with substance abuse/dependency and/or their collaterals; SED diagnosis; other DSM IV diagnosis; and behavior problems. Services are provided in a variety of settings including clinic sites, schools, detention centers and Head Start centers.

Group Work - Staff facilitate groups for children and/or adolescents with the following:Presenting issues: COA (Children of Alcoholics), substance abuse/dependency, behavior or learning problems, poor self-esteem, depression. Staff also conducts parenting groups for those parents with deficient skills or who have a child with a mental health or substance abuse problem.

Targeted Case Management - This service is provided to children/adolescents who have a DSM IV diagnosis and/or severe delays in emotional development, require significant intervention from other agencies, or experience significant stress from some outside source, or are at risk of these problems.

In-Home Services - Intensive therapy and case management services are primarily provided in the home to families in which there is a child that is at risk of an out-of-home placement due to emotional problems, and for whom less restrictive options have been attempted without success. Referrals are only accepted through FAPT Teams.
