CASE MANAGER - SPECIAL DELIVERIES PROGRAM

TYPE OF WORK: Be part of an innovative new program working with families and young children. Special Deliveries works with parents and children under 5 years old and uses a home visitation model of practice to increase parenting skills. This positions will provides individualized Case Management services for parents and children identified as eligible for Case Management services due to emotional problems, substance abuse issues, developmental delay, or mental retardation. Staff will coordinates services and resources needed by the parent/child to maintain or return to community living.

Cell phone, laptop, weekly licensure supervision, mileage, and great working environment provided.

QUALIFICATIONS:
Graduation from an accredited college or university with a Bachelor’s degree in Social Work or human services related field, with at least one year of experience in a setting working with children or adults with needs in the areas of  mental health, substance abuse, developmental delay, or mental retardation. Demonstrated knowledge of case management/social work practices and resources available to help consumers in the community. Valid Virginia driver’s license and safe driving record required.

STARTING SALARY: $
28,874
