TYPE OF WORK:
Provides needs
assessments, linking to resources, and monitoring
services for individuals with serious mental illness
and/or intellectual disabilities receiving services
through the mental retardation Medicaid waiver program.
This includes individuals diagnosed MH/ID, MH/SA, and
MH/ID/SA living in the community and/or an Assisted
Living Facility (ALF). Coordinates services and/or
resources needed by the consumer to maintain and/or
return to community living. Monitors service delivery to
ensure consumer needs are met. Ensures documentation of
services meets requirements. Provides some assistance
with transportation, community integration, and money
management.
QUALIFICATIONS:
Requires Bachelor’s
degree in human services or related field and at least
one year’s experience working with individuals with
mental illnesses. Must have demonstrated knowledge of
case management/social work practices and resources
available to help consumers in the community.
Knowledge of Medicaid guidelines as they pertain to
State Plan Option is helpful. Demonstrated ability to
communicate clearly and concisely (written and oral).
Initiative, resourcefulness, and ability to work
independently as well as part of a team. Basic
knowledge of personal computers and word processing.
Valid driver's license and an acceptable driving record
required.
STARTING SALARY:
$28,874
